Beacon of HOPE Wellness Committee: Discover Hope PACE
Hope PACE, a program of all-inclusive care for the elderly, is a comprehensive, coordinated healthcare program designed to meet the social, emotional and practical challenges of older adults. This innovative program helps seniors live safely in the place they call home.
Comprehensive care plans are customized to meet each client’s needs and include services such as medical and dental care, therapies, home and personal care, medications, medical supplies and essential transportation. For isolated or lonely seniors, warm meals and social activities at state-of-the-art Hope Care Centers are a welcome change.
To be eligible, participants must live in Charlotte, Collier or Lee counties and be 55 years of age or older. Our Hope Care Centers are conveniently located in Fort Myers, Lehigh Acres, Port Charlotte and Naples.
