The office is closing effective Jan. 10, 2013, due to health issues and retirement. We appreciate your patronage; it has been a pleasure to serve your dental needs.
At your request, you can request copies of your permanent records, and these can made available for your newly chosen dentist. If you wish to request copies of your dental records, please contact the office prior to the closing date. We will need you to sign a release for your records.
It is with a mixture of sadness and anticipation that I announce the closing of my dental practice upon my retirement in January 2013. I have practiced dentistry in this community for 24 years, and have chosen to bring my active practice to a close to spend more time with my family.
I am very fortunate to have practiced dentistry in a community that supports my efforts so greatly. I have enjoyed our friendship and your commitment to oral health over the years.
