If you live on Pine Island or west of Matlacha Isles and have a child that will be age 5 on or before Sept. 1, 2010, you can come to the office at Pine Island Elementary School from 8:30 a.m. to 3:30 p.m. to pick up paperwork to get your child registered for the next school year. Registration requirements:

— Original birth certificate

— Health examination (physical)

— Social Security card (if available)

— Florida Certificate of Immunization (original blue card)

— Proof of residence

— Proof of custody, if child is not living with both natural parents

— Driver’s license
